Sometimes you know you need some support, but you aren't sure where to start. Community Connector helps older adults explore their options and connect with the people, programs, services and activities that can support health, wellbeing and independence.
Our Community Connector uses social prescribing and personalized navigation to help you take the next step.

What is social prescribing?
Social prescribing is a person-centered approach that connects people with non-medical supports in their community. Rather than focusing only on a medical concern, we look at the whole person—what matters to you, what you enjoy, what challenges you are experiencing and what could help you feel healthier and more connected.
You don't have to navigate it alone.
Finding the right health, social or community service can be confusing. There may be different organizations, eligibility requirements, applications or systems to navigate. Your Community Connector can help you understand what is available, identify where to start and connect you with appropriate resources. If Lake Country Health doesn't provide the service you need, we'll help you find out who does.

How it Works

Contact us directly or be referred to the Community Connector.
2. Listen
We take time to understand what is happening, what matters to you and what you would like to change.
3. Explore
Together, we identify programs, services, activities and resources that may help.
4. Navigate & Connect
We help you take the next step and connect with the appropriate support.
5. Follow Up
We check in to see how things are going and help with next steps if needed.
