Nav CARE Program

Nav-CARE (Navigation: Connecting, Advocating, Resourcing, Engaging), is a program offered at no cost that supports people living with declining health to live well at home for as long as possible. Specially trained volunteer navigators help persons and their families access resources and services in their community, while providing companionship and emotional support.

How this service works for individuals, care-partners & families
  1. Complete an Intake Form. You complete a short intake form and matching survey to help us pair you with a Volunteer that is a good fit for your needs and personality. 

  2. Connect with our Coordinator. You can start the intake process by downloading the form in the hyper link above and submitting via email to, caregiver@lakecountryhealth.ca Alternatively, you can call our Caregiver Support Coordinator for support with this process via their direct line, 250-306-2540.

  3. Schedule a Time to Connect. Once an intake has been completed and filed the Coordinator will set up a time to connect with you to talk about your situation and assess your needs by conducting our matching survey.

  4. Meet your Volunteer Navigator. Once your needs have been assessed, our Coordinator will match you with a Volunteer Navigator and set up a meeting. For the initial meeting our Coordinator will be present to facilitate connection, clarify the scope of this program, and everyone better understand your needs, and expertise of the volunteer. 

  5. Receive on going support. After your initial meet and greet your Volunteer Navigator will set up times to connect with you (and your care partner) on a regular basis. Your Volunteer Navigator will receive support and guidance from our Coordinator. 

What to Expect

Our Volunteer Navigators complete thorough training on how to support people experiencing declining health, and their care-partners. At the beginning of each visit your Volunteer Navigator will check-in with you to better understand what your priorities are for the visit and your time together. Meetings between your and Volunteer can take place in-person, online, or over the phone. Frequency of meetings are between one and four times a month with meetings lengths varying from thirty minutes to two hours- depending on your specific needs. 

 

These meetings can be scheduled on an as needed basis or set up to take place on a reoccurring day and time.

What to Expect

These one to one support meetings can be scheduled as a phone call, face time, or in-person get together. Our Caregiver Support Coordinator is well versed in the landscape of support for unpaid caregivers within the Okanagan Valley. They take the time to listen to your unique story, help you to better understand your priorities and needs, and apply their knowledge of local and provincial resources to your specific situation. The goal of these calls is to help you feel clear, supported, and seen.
