One on One Caregiver Support
1 in 4 Canadians are unpaid caregivers. Majority of people never expect to step into this role, until someone they love needs help. This journey can feel overwhelming, isolating, and at times exhausting. We believe all caregivers deserve support - at every stage of their journey. Lake Country Health provides free guidance and support for people that are caregiving their friends or family who are aged 55 years and older.
How this service works

Complete an Intake Form. You complete a short intake form with contact information, basic details about the person you care for, and the Activities of Daily Living (click this link to see glossary definitions), that you assist with.

Connect with our Coordinator. You can start the intake process by downloading the form in the hyper link above and submitting via email to, caregiver@lakecountryhealth.ca Alternatively, you can call our Caregiver Support Coordinator for support with this process via their direct line, 250-306-2540.

Schedule a Time to Connect. Once an intake has been completed and filed the Coordinator will set up a time to connect with you to talk about your situation and assess your needs.
What to Expect
These one to one support meetings can be scheduled as a phone call, face time, or in-person get together. Our Caregiver Support Coordinator is well versed in the landscape of support for unpaid caregivers within the Okanagan Valley. They take the time to listen to your unique story, help you to better understand your priorities and needs, and apply their knowledge of local and provincial resources to your specific situation. The goal of these calls is to help you feel clear, supported, and seen.
These meetings can be scheduled on an as needed basis or set up to take place on a reoccurring day and time.
